Applicants For Pre-Employment
All applicants scheduled for a pre-employment psychological evaluation will need to complete a background questionaire. The questionaire can be downloaded below from our website and printed out for completion. The pre-employment evaluation is an interview process in addition to a psychological evaluation. It is suggested that the applicants dress appropriately and come prepared for the full evaluation. Also, a copy of a valid driver's license or military identification is required. All public safety pre-employment psychological evaluations begin at 8:00 a.m. This is a lengthy and extensive process often lasting until 3:00 p.m. or later.
Polygraph Booklet
All applicants scheduled for a pre-employment Polygraph will need to complete the Polygraph Booklet. The Booklet can be downloaded below from our website and printed out for completion.
Consent Forms
Sage Cognitive Solutions are in compliance with all applicable state and federal regulations regarding confidentiality and privacy. In many cases the evaluations being completed are for a third party (i.e. Police, Fire Department, CPS) and there is limited confidentiality. In all cases, the confidentiality and privacy issues for the type of evaluation being done are spelled out on the consent form which is provided for review and signature before the evaluation begins.
In the public safety evaluations done at Sage Cognitive Solutions, our client is the department sending the person for an evaluation. Sage Cognitive Solutions is not providing mental health services to the person who appears for these evaluations, but instead providing an administrative evaluation. The results of these administrative evaluations will be shared with the department who sent the person being evaluated. It will not be shared beyond that without further consent by the person and/or the department.
